• Valid for new referrals & residents that have not been in prior contact (or registered) with SeniorCareHomes.Com.
• Provide us with the Resident’s First and Last Name, and the information for the main point of contact.
• The person you signed a Move-in-Contract with the care facility selected.
• Care options must be provided by SeniorCareHomes.Com
• The selected facility must not have prior contact with resident or referring party.
• Minimum stay in the facility must be at least 60 days.
• Valid for Assisted Living, Memory Care or Independent Living and Private Pay (Out of Pocket) only.
• Minimum rent plus care is $3,000/month to qualify.
• Gift card will only be issued if/when SeniorCareHomes.Com is compensated by the facility.
• Gift card will be issued 1-3 business days from the time SeniorCareHomes.Com receives compensation from the facility.
• Program not valid for admissions covered by: Medicare, Medicaid or as otherwise prohibited by law, respite/short-term stays fewer than 60 days, skilled nursing, or any government subsidized programs.
• You agree our Partners may use automated calling technology to contact you by call or text message at the telephone number provided, even if you previously registered on a Do Not Call registry. This consent is not required to obtain our or our Partners’ services or products.
