Overview of Intermediate Care 

Intermediate care provides active therapy and treatment that gives an opportunity for recovery through structured individual care plans. It is a combination of health and social care plans that aim to provide patients with an opportunity to recover faster from an illness or minor injury.

Intermediate Care Facilities are health facilities that provide inpatient care to those who are in need of skilled nursing supervision and supportive care, but do not require continuous nursing care. They are used to avoid unnecessary admission to a hospital or premature admission to an assisted living or other long-term residential care facilities.

This type of care is suited for patients who want to maximize their independence while recovering from a short-term illness before going home. Typically, care is given to patients who need care for as little as one week, but not longer than 6 weeks.

Intermediate Care facilities are state licensed facilities that provide a wide range of medical, social, recreational and support services for older adults. These care facilities are associated with nursing homes, skilled nursing facilities and hospitals and are also known as Rest Homes or Care Homes with Nursing Supervision.

Specialized care facilities for the developmentally disabled adults who need intermediate care are available. Here are the types of available:

1. Developmentally Disabled also known as ICF-DD, is a skilled nursing facility that provides intermediate care and support services to developmentally disabled clients whose primary need is for developmental services and have a recurring but intermittent need for skilled nursing services.

2. Developmentally Disabled Habilitative also known as ICF-DDH, is an intermediate care facility with a capacity of 4 to 15 beds that provides 24-hour personal care, habilitation, developmental, and supportive health services to 15 or fewer developmentally disabled person with intermittent recurring needs for nursing services, but have been certified by a physician and surgeon as not requiring availability of continuous skilled nursing care.

3. Developmentally Disabled Nursing Facility also known as ICF-DDN, is a type of intermediate care facility with a capacity of 4 to 15 beds that provides 24-hour personal care, developmental services, and nursing supervision for developmentally disabled person who have intermittent recurring needs for skilled nursing care but have been certified by a physician and surgeon as not requiring continuous skilled nursing care. This intermediate care facility shall serve medically fragile persons who have developmental disabilities or demonstrate significant developmental delay that may lead to a developmental disability if not treated.
